I am Filling Out an HRP Form for the FIRST TIME

Creating an HRP Form involves a short, two-stage process. The first stage is to login.

If you have previously registered with the Florida Region, please login by clicking on Member Login on the right side of the screen. If you do not remember your password, just click on Lost Your Password under the Member Login link.

If you have not registered with the Florida Region, please Sign Up and create an account on the Florida Region website. Once you haved created an account, please click on HRP Form again.

In the second stage you will complete the HRP Form. When you are finished, click the "submit" button and it will forward your form to us immediately.


I Want to UPDATE My HRP Form

If you registered with the Florida Region prior to 8/1/2011, please login by clicking on Member Login on the right side of the screen. If you do not remember your password, just click on Lost Your Password under the Member Login link.

If you have not registered with the Florida Region, please Sign Up and provide some basic contact information. Once you haved created an account, please contact HRP to have your current HRP Form linked to your Florida Region account.

The easiest way to contact HRP is to click on Contact located at the top menu of this screen. Or, send an email to hrp@naflorida.org. Please include your name and area in the email.


I Want to UPDATE My HRP Form - Optional

If you do not wish to contact HRP to have your current HRP form linked, you have the option to fill out a new HRP Form. Your old form will be deleted from the system. You will still need to Login or Sign Up to create an account on the Florida Region website before you can create or update an HRP form..


Note: Prior to 8/1/2011 HRP Forms could be submitted without having to login. Enhancements were made giving members the capability to update their HRP Form. In order to accomplish this in a secure environment, it was necessary to link your HRP Form to your Florida Region account.

If you filled out an HRP Form prior to 8/1/2011 and it does not appear for updating, it is still in the database. Your HRP Form will need to be linked to your Florida Region account. This is a simple process, but you will need to contact us and we will get your form linked.
